Leave Requests
- Submit non-emergency leave requests to the Training Director and Program Assistant at least 45 days in advance of the first day of planned leave. Non-emergent leave requests include, but are not limited to vacations, scheduled health procedures, education, and/or administrative leave. All requests submitted less than 45 days will be reviewed on a case-by-case basis with careful consideration to the potential negative impacts on the needs and requirements of the program, patient satisfaction, and operational goals and objectives.
- For emergent leave requests, please inform the Training Director and Program Assistant as soon as known. Although we do not routinely ask for a doctor’s note to confirm illness, we may request one if an intern’s pattern of taking sick time is of concern to the program. Leave will not be approved during the first or last week of the internship program. Exceptions to this policy will only be made under extraordinary circumstances.
Record Retention
The internship program permanently maintains records of the interns’ training schedules, evaluations, and diplomas. All grievances and complaints are also stored appropriately. All materials are stored on private cloud-based service.
